Patient navigators serve a critical role in communities across the nation. They help eliminate barriers to diagnosis and treatment and exponentially increase a patient’s chances of getting the proper care.

  • Ask your Medical Provider

If you have been diagnosed with a chronic disease, ask your doctor if they can connect you to a patient navigator. If they can’t, contact the Harold P. Freeman Patient Navigation Institute at 1-877-369-0902 or email info@hpfreemanpni.org to request assistance.

  • Support Training

Your generous donation to our scholarship fund helps us train patient navigators in the community so they can continue to make a difference in patients’ lives. The cost to train one individual at the Patient Navigation Institute is approximately $1,500.

The Harold P. Freeman Patient Navigation Institute Inc. is a 501(c)(3) tax-exempt charitable, non-profit organization. All donations are tax deductible to the extent permitted by law.

We are grateful to Accenture for helping us create this donation mechanism as part of its Skills to Succeed corporate citizenship initiative. Accenture's focus on providing people with the skills to become patient navigators reflects its goal to equip 250,000 people by 2015 with the skills to get a job or build a business.
