Patient navigators serve a critical role in communities across the nation. They help eliminate barriers to diagnosis and treatment and exponentially increase a patient’s chances of getting the proper care.

 

• Ask your Medical Provider

 

If you have been diagnosed with a chronic disease, ask your doctor if they can connect you to a patient navigator. If they can’t, contact the Harold P. Freeman Patient Navigation Institute at 1-877-369-0902 or email info@hpfreemanpni.org to request assistance.

 

• Support Training

 

Your generous donation to our scholarship fund helps us train patient navigators in the community so they can continue to make a difference in patients’ lives. The cost to train one individual at the Patient Navigation Institute is approximately $1,500.
